Job summary

Enhabit Home Health & Hospice in Florida is seeking a licensed professional nurse or physical therapist to review and approve patient information submitted by LPs during start of care, re-certification, resumption of care or evaluation visits.

The role partners with branch leadership to educate LP staff on documentation requirements to meet regulatory standards and schedules one-on-one training when needed; in-office attendance is essential to support supervision, teamwork and patient care

Qualifications

Graduate of an approved school of professional nursing or physical therapy.
Current valid clinical license per state requirements.
Minimum one year of experience in Medicare home health or hospice.
CPR certification and valid state driver license with insurance.
Basic technology skills including mobile device use.

Responsibilities

Review and approve patient information submitted by LPs during start of care, re-certification, resumption of care, or evaluation visit.
Partner with branch leadership to provide ongoing education and training to LPs to meet regulatory standards.
Conduct one-on-one training with LPs when necessary.
Promote quality patient care and maintain relationships with referral sources, patients, caregivers and staff.
