Job summary

Enhabit Home Health & Hospice is seeking a licensed professional nurse or physical therapist to review and approve patient information across care milestones. You will train branch LPs, ensure documentation meets standards, and collaborate with leadership to improve patient outcomes.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Valid clinical license per state requirements.
• Minimum of one year of experience.
• Experience in Medicare home health or hospice setting.
• CPR certification and a valid state driver's license; auto liability insurance.

Responsibilities

• Review and approve patient information submitted by LP during start of care, re-certification, resumption, or evaluation visit.
• Provide ongoing education and training to branch LPs to meet regulatory standards.
• Meet one-on-one with LPs for in-depth training when necessary.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and employees.
