Job summary

Enhabit Home Health & Hospice is seeking a licensed professional (nurse or physical therapist) to review and approve patient information across start of care, recertification, and evaluation visits. The role partners with branch leadership to educate LPs on documentation and regulatory standards, with in-person training as needed.

The ideal candidate has a nursing or physical therapy degree, an active license, CPR certification, a valid Georgia driver's license, and Medicare home health

Qualifications

• Graduate of an approved nursing or physical therapy program with a valid clinical license.
• Minimum one year of experience in Medicare home health or hospice.
• CPR certification and a valid driver's license with automobile liability insurance.

Responsibilities

• Review and approve patient information submitted during start of care, recertification, resumption of care, or evaluation visit.
• Provide ongoing education and training to branch LPs on documentation requirements to meet regulatory standards.
• Meet with LPs for in-depth training when necessary.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and staff.
