Job summary

Enhabit Home Health & Hospice is seeking licensed professionals to review patient information and ensure regulatory readiness during start of care, re-certifications, and evaluations. The role involves collaborating with branch leadership to educate LP staff and provide focused training to maintain high standards of documentation and patient care.

The position requires an active nursing or therapy license, CPR certification, and a valid state driver license.

Qualifications

• Must be graduate of an approved school of professional nursing or physical therapy.
• Valid RN license or applicable professional credential.
• CPR certification required.
• State driver license and automobile insurance required.
• Strong communication and technology skills including basic Microsoft Office use.

Responsibilities

• Review and approve patient information submitted by the LP during start of care, re-certification, resumption of care or evaluation visit.
• Partner with branch leadership to provide ongoing education and training to all branch LPs to ensure documentation requirements meet regulatory standards and keep staff updated on changes.
• Meet one-on-one with LPs to provide in-depth training when necessary.
• Promote quality patient care and maintain stable relationships with referral sources, patients, caregivers and employees.
