Job summary

Enhabit Home Health & Hospice is seeking a qualified licensed professional to review patient information during start of care, recertification, or evaluations. You will collaborate with branch leadership to educate LPs, provide targeted training, and uphold regulatory standards while promoting high-quality patient care and strong relationships with referral sources.

The role requires an active nursing or physical therapy license, strong English communication, and tech proficiency with Microsoft

Qualifications

• Licensed professional nursing or physical therapy required with active license.
• Experience reviewing patient information and training LPs preferred.
• Strong technology and documentation skills; capable with mobile devices.

Responsibilities

• Review and approve patient information from LP during care processes.
• Partner with branch leadership for ongoing education and training.
• Provide in-depth training and ensure compliance with regulatory standards.
• Maintain quality patient care and strong relationships with referrals and staff.
