Job summary

Enhabit Home Health & Hospice is seeking a licensed professional (nurse or physical therapist) to review and approve patient information for start of care, recertifications, and care evaluations.

The role partners with branch leadership to educate LPs on documentation requirements to meet regulatory standards and to provide one-on-one training when needed. A focus on quality patient care and strong relationships with referral sources is essential.

Qualifications

• Required graduate of an approved school of professional nursing or physical therapy.
• Required to have and maintain a valid clinical license, per state requirements to complete this job in accordance with applicable board’s rules.
• + One year of experience required.
• + Medicare home health or hospice setting experience preferred.
• + CPR certification required.
• + Valid state driver license and automobile liability insurance required.
• + Preferred experience with packet review / quality improvement and with coding.
• + Basic technology skills including operation of a mobile device.
• + Experience with Homecare Homebase (HCHB) preferred.
• + Dependable transportation to travel as needed.

Responsibilities

• Review and approve patient information submitted by the LP during start of care, recertification, resumption of care, or evaluation visit.
• Partner with branch leadership to educate and train LPs on documentation to meet regulatory standards.
• Provide one-on-one training for LPs when necessary.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers, and staff.
