Job summary

Enhabit Home Health & Hospice, a leading national provider, seeks a licensed professional (RN or PT) to review patient information during care starts, recertifications, and evaluations. You will train branch LPs on documentation and regulatory updates to ensure compliance and high-quality care.

The role requires Medicare home health experience, CPR, a valid driver license, and reliable transportation. This on-site position offers a collaborative culture, career development, and strong benefits

Qualifications

• Required graduate of an approved school of professional nursing or physical therapy.
• Must maintain a valid clinical license per state requirements.
• Minimum of one year of experience in Medicare home health or hospice.
• CPR certification required.
• Must have a valid state driver license and automobile liability insurance.
• Basic technology skills including use of mobile devices.

Responsibilities

• Review and approve patient information submitted by the Licensed Professional (LP) during start of care, recertification, resumption of care, or evaluation visits.
• Partner with branch leadership to educate and train LPs on documentation requirements to meet regulatory standards.
• Provide one-on-one training when necessary.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and employees.
