Job summary

Enhabit Home Health & Hospice is looking for a licensed professional (RN or PT) to review patient information at start of care and during care updates. You will train staff on documentation, ensure regulatory compliance, and foster quality patient care.

Strong communication and tech skills are required, with in-office attendance essential to supervise and coordinate teams. Applicants should have a nursing or physical therapy background, a current license, CPR, a valid driver license, and

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Maintain a valid clinical license per state rules.
• Minimum of one year of experience; Medicare home health or hospice preferred.
• CPR certification required; valid state driver license and auto liability insurance.
• Fluent in English with strong oral/written communication.
• In-office attendance is essential for supervision and teamwork.

Responsibilities

• Review and approve all patient information submitted by the Licensed Professional during start of care, recertification, resumption, or evaluation visit.
• Partner with branch leadership to provide ongoing education and training on documentation and standards.
• Provide one-on-one training when needed and promote quality patient care.
• Maintain relationships with referral sources, patients, caregivers and employees.
