Job summary

Enhabit Home Health & Hospice is seeking a qualified professional to review and approve patient information during starts of care, recertifications, and evaluations, working with LPs and branch leaders to ensure accuracy and regulatory compliance.

The role emphasizes training LPs, delivering ongoing education, and maintaining high standards of patient care. A valid license and CPR certification are required, along with strong Microsoft Office skills and reliable transportation.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• CPR certification required.
• Valid state driver license and automobile liability insurance.
• Experience in Medicare home health or hospice setting preferred.
• Technology skills including Microsoft Office and mobile device operation.

Responsibilities

• Review and approve patient information during start of care, re-certification, resumption of care, or evaluation visit.
• Partner with branch leadership to educate LPs and ensure documentation meets regulatory standards.
• Provide one-on-one training when necessary and promote high-quality patient care.
