Job summary

Enhabit Home Health & Hospice is seeking a qualified licensed professional (RN or PT) to review patient information during care starts, recertifications and evaluations. The role works with branch leadership to educate LPs and ensure compliance with documentation standards.

The position requires valid licensure, CPR, a driver license, and Medicare home health or hospice experience. In-office attendance is essential to support supervision and teamwork.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Must hold and maintain a valid clinical license per state requirements to practice.
• Minimum of one year of experience.
• Experience in Medicare home health or hospice setting preferred.
• CPR certification is required.
• Must hold a valid state driver license and automobile liability insurance.
• Proficiency with basic technology; MS Office suite (Word, Excel).
• Experience with Homecare Homebase (HCHB) preferred.

Responsibilities

• Review and approve patient information submitted by the Licensed Professional (LP) during start of care, recertification, resumption of care, or evaluation visit.
• Partner with branch leadership to provide ongoing education and training to all branch LPs on documentation requirements and regulatory standards.
• Provide one-on-one in-depth training as needed and promote quality patient care; maintain relationships with referral sources, patients, caregivers and staff.
