Job summary

Enhabit Home Health & Hospice is seeking a licensed professional to review and approve patient information during start of care, re-certification, resumption of care, and evaluation visits. You will partner with branch leadership to educate LP staff and ensure documentation meets regulatory standards.

Role emphasizes training, quality patient care, and strong relationships with referral sources, patients, caregivers and employees. In-office attendance is essential outside remote positions.

Qualifications

• Oral communication, written communication and active listening are essential.
• Proficiency with Microsoft Office (Word, Excel) is required.
• License to practice and regulatory compliance must be maintained.
• Experience in Medicare home health or hospice settings is preferred.
• CPR certification is required and up-to-date.

Responsibilities

• Review and approve patient information submitted by LP during care starts and recertifications.
• Collaborate with branch leadership to educate and train LP staff on documentation requirements.
• Provide one-on-one in-depth training when needed.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and employees.
