Job summary

Enhabit Home Health & Hospice is seeking a licensed professional (RN or PT) with Medicare home health or hospice experience to review patient information during start of care and certifications. You will train branch LPs on documentation and regulatory standards, and foster strong relationships with patients, caregivers and referral sources.

The role requires a valid license, CPR, a valid driver license, and basic tech skills, with in-office attendance emphasized to achieve organizational goals.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Valid clinical license per state requirements to practice (NLP/LP).
• Minimum 1 year experience in Medicare home health or hospice.
• CPR certification required.
• Valid state driver license and automobile liability insurance.
• Basic technology skills and mobile device proficiency.

Responsibilities

• Review and approve patient information submitted by LP during start of care, recertification, resumption of care or evaluation visit.
• Provide ongoing education and training to branch LPs to ensure documentation meets regulatory standards.
• Offer one-on-one in-depth training when needed.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and staff.
