Job summary

Enhabit is seeking qualified professionals for home health and hospice services. The role involves reviewing and approving patient information submitted by Licensed Professionals during care initiation and recertification, while partnering with branch leadership to ensure proper documentation and regulatory compliance.

The position requires a nursing or physical therapy license, CPR certification, and at least 1 year of relevant experience.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Must hold a valid clinical license to practice per state requirements.
• Minimum 1 year of experience in home health or hospice recommended.
• CPR certification required.
• Valid state driver license and automobile liability insurance required.
• Remote work is not allowed for this role unless designated; in-office attendance is essential.

Responsibilities

• Review and approve patient information submitted by the Licensed Professional (LP) during start of care, recertification, resumption of care, or evaluation visit.
• Partner with branch leadership to educate and train LPs on documentation requirements to meet regulatory standards.
• Provide one-on-one training to LPs when necessary.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and employees.
