Job summary

Enhabit Home Health & Hospice, a nationwide home-based care provider, is seeking qualified healthcare professionals to review and approve patient information and provide ongoing education to licensed professionals. The role emphasizes adherence to regulatory standards and strong collaboration with branch leadership.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Valid clinical license per state requirements.
• Minimum of one year of experience.
• Experience in Medicare home health or hospice setting preferred.
• CPR certification required.
• Valid state driver license and automobile liability insurance required.
• Preferred experience with packet review / quality improvement.
• Preferred experience with coding.
• Basic technology skills including operation of a mobile device.
• Preferred experience with Homecare Homebase (HCHB).
• In-office attendance essential for non-remote positions.
• Fluency in English; oral and written communication; active listening.
• Proficient with Microsoft Office (Word and Excel).

Responsibilities

• Review and approve patient information submitted by the Licensed Professional during start of care, recertification, resumption of care, or evaluation visit.
• Partner with branch leadership to provide ongoing education and training to all branch LPs on documentation requirements.
• When necessary, meet one-on-one with LPs to provide in-depth training.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and employees.
