Job summary

Texas Health and Human Services Commission seeks a QIDP II at Mexia State Supported Living Center to oversee ISP processes, coordinate IDT meetings, and monitor treatment plans for individuals with intellectual disabilities. You will ensure timely assessments, involve the individual and LAR, and lead staff training on supports and behavior management.

Qualifications

• Bachelor’s degree in psychology, sociology, social work, special education, rehabilitation counseling, or a related human services field.
• At least one year experience working or volunteering directly with persons with intellectual disabilities or other developmental disabilities.

Responsibilities

• Orchestrates and oversees the ISP process for all individuals on the assigned caseload.
• Facilitates IDT meetings and documents ISP preparations and annual reviews.
• Ensures assessments and evaluations occur to determine functioning and needs.
• Coordinates participation of individuals and LAR in developing supports and services.
• Monitors ISP implementation and modifies plans as progress or health status changes.
• Trains staff on service delivery and behavior management; leads interdisciplinary guidance.
