Job summary

The candidate will ensure regulatory compliance, document progress, and supervise staff while participating in a rotating on-call schedule to provide around-the-clock support.

Qualifications

• Requirements include strong communication and case management abilities.
• Experience with individuals with intellectual/developmental disabilities is preferred.
• Knowledge of state and federal regulations governing residential services is required.

Responsibilities

• Lead and coordinate the ISP process for residents in Shore’s Residential homes.
• Oversee development, implementation, and monitoring of active treatment plans and program services.
• Ensure services comply with state and federal regulations and organizational policies.
• Serve as a case manager, advocate, and liaison for residents, families, and service providers.
• Participate in rotating on-call schedule to provide 24/7 shift support for residential homes.
• Collaborate with interdisciplinary teams to assess needs and adjust care plans.
• Document all services, interventions, and progress accurately and timely.
• Provide guidance and supervision to direct care staff regarding treatment plans and regulatory compliance.
