Job summary

Texas Health and Human Services is seeking a Qual Intel Disability Prof III to oversee ISP planning for individuals at the Richmond facility. The role leads the IDT, coordinates interdisciplinary services, and ensures documentation, evaluations, and program revisions are completed timely.

The QIDP III will train staff, monitor progress, and collaborate with the Legally Authorized Representative, maintaining compliance with ICF/IID standards and the Settlement Agreement.

Qualifications

• Bachelor’s degree from an accredited college or university in a human services field.
• At least 1 year of experience working directly with persons with intellectual or developmental disabilities.
• Knowledge of the IDT process and person-centered planning.
• Ability to coordinate programmatic and clinical services and supports.
• Proficiency in documenting, reporting, and maintaining records.
• Strong communication and collaborative skills.
• Familiarity with PBSPs and related plans.

Responsibilities

• Oversee the ISP process for all individuals on the caseload, including ISP preparation and annual meetings.
• Facilitate and document IDT meetings with participation from relevant staff and family/LAR.
• Ensure assessments determine functioning, preferences, strengths, needs, and recommendations.
• Coordinate development and delivery of needed supports and services.
• Monitor and adjust ISP plans to meet progress and changes in behavior/health.
• Train and coach staff on service delivery and documentation.
• Ensure proper protections and apply human rights processes as needed.
• Maintain electronic records with timely, compliant documentation.
• Support disaster response, COOP, and OT duties as assigned.
