Job summary

The Texas Health and Human Services Commission (HHSC) invites qualified candidates to join as a QIDP II at Mexia State Supported Living Center. The role centers on coordinating the ISP process, leading the IDT, and ensuring individualized plans reflect the person’s needs, preferences, and strengths.

Candidates should demonstrate strong organizational skills, experience with ICF/IID standards, and the ability to work with individuals who may have aggressive or self-injurious behaviors.

Qualifications

• Bachelor’s degree in psychology, sociology, social work, special education or related field required.
• Experience coordinating ISP processes and IDT meetings preferred.
• Strong written and verbal communication and data-tracking abilities.
• Ability to work with individuals who have complex needs and challenging behaviors.

Responsibilities

• Orchestrates ISP processes for all individuals on caseload and facilitates IDT meetings.
• Monitors and documents progress, and ensures timely follow-up on recommendations.
• Coordinates with staff and LARs to deliver needed supports and services.
• Trains staff and provides leadership on assessment and behavior support.
