Job summary

The incumbent will ensure timely documentation, participate in other QIDP-related meetings, and maintain a limited caseload while supporting education and coordination across departments.

Qualifications

• Bachelor’s degree from an accredited college or university with a major in psychology, sociology, social work, special education, rehabilitation counseling, or a related human services field.
• At least two years of experience working directly with persons with intellectual disabilities or other developmental disabilities, including experience as a QIDP.
• Experience coordinating and facilitating group meetings.

Responsibilities

• Facilitates ISP meetings and prepares prep work for ISPs.
• Leads interdisciplinary team discussions and coordinates actions toward individualized goals.
• Ensures documentation is timely, accurate, and compliant with policy.
• Coordinates across departments to support ISP processes and service planning.
• Maintains person-centered practices in daily activities and service delivery.
• Provides coverage for operational needs and supports disaster response readiness.
