Job summary

Health & Human Services Commission (HHSC) is seeking a Qualified Intellectual Disability Professional II to oversee ISP processes, coordinate IDT meetings, and supervise implementation of supports for individuals at the Brenham SSLC. The role requires coordinating with LARs, documenting progress, and ensuring adherence to ICF/IID standards.

Responsibilities include leading ISP planning, ensuring timely follow-up, and providing staff training on person-centered approaches and behavior supports.

Qualifications

• Bachelor’s degree in a related human services field.
• Experience with IDT meetings and ISP processes preferred.
• Knowledge of ICF/IID standards and related regulations.

Responsibilities

• Orchestrates the ISP process for all individuals on the caseload and coordinates IDT meetings.
• Ensures assessments determine current functioning, preferences, strengths, and needs.
• Coordinates participation with the individual and LAR for needed supports.
• Trains staff and coaches on service delivery and behavior management.
• Monitors ISP implementation and documents monthly review outcomes.
• Ensures compliance with applicable federal and state standards and rights processes.
