Job summary

Texas Health and Human Services Commission (HHSC) seeks a Qualified Intellectual Disability Professional II in Brenham, TX. The role oversees the ISP process, coordinates the IDT, and ensures individualized supports align with preferences and needs.

Strong organizational and documentation skills are essential. The position requires a bachelor’s degree in a human-services field and at least 1 year of relevant experience.

Qualifications

• Bachelor’s degree in a prescribed human services field is required.
• At least 1 year of experience working with persons with intellectual/developmental disabilities.
• Experience with ISP processes and IDT facilitation is preferred.
• Ability to operate electronic records systems and document thoroughly.

Responsibilities

• Orchestrates the ISP process for all individuals on the caseload and leads IDT meetings.
• Ensures assessments determine functioning, preferences, and needs.
• Coordinates with LARs and staff to deliver supports and services.
• Monitors ISP implementation, documents progress, and revises plans as needed.
• Trains staff on service delivery and behavior management strategies.
• Maintains compliance with ICF/IID standards and applicable settlements.
