Job summary

Hartford HealthCare at Home is seeking a Quality Assurance & Performance Improvement (QAPI) Specialist to drive clinical documentation and outcomes through data-driven processes. You will compile trends, report quality data across patient care, safety, risk management, infection control, outcomes, and customer satisfaction, and lead Performance Improvement Plans.

The role requires 3 years of certified home health experience, CT licensure, ability to travel to branches, and strong communication

• Value Based Purchasing, PDGM, CAHPS-preferred.
• Licensure, Certification, Registration.
• CT Licensure in field of specialty.
• Knowledge of federal and state regulatory requirements.

Responsibilities

• Develop interventions to impact quality scores.
• Participate in delegated groups to improve quality metrics.
• Review corporate indicators data and other comparative data.
• Facilitate use of evidence-based guidelines to reduce variation in practice.
• Ensure timely and accurate submissions to payors and regulators.
