Job summary

Hartford HealthCare at Home seeks a QAPI Specialist to guide clinical documentation improvement and quality outcomes. You will compile trends, report quality data, and lead performance improvement initiatives across patient care, safety, infection control, and customer satisfaction.

This role requires 3 years of certified home health experience, CT licensure, and ability to travel to branches. You will facilitate PI teams, ensure regulatory compliance, and educate staff on evidence-based

Qualifications

Three years of certified home health experience required.
Value Based Purchasing, PDGM, CAHPS-preferred.
Licensure, Certification, Registration.
CT Licensure in field of specialty.
Knowledge of federal and state regulatory requirements.
Excellent written and verbal communication skills.
Travel within the United States may be necessary.

Responsibilities

Assist with process improvement plans and record reviews.
Compile trends and reports quality data in patient care, safety, risk management, infection control, outcomes and customer satisfaction.
Lead development of Performance Improvement Plans.
Ensure compliance with federal and state regulations and payors.
Travel to branches as needed to support sites.
Facilitate PI teams using quality improvement methodologies.
Review and analyze data to drive performance improvements.
