Job summary

Hartford HealthCare at Home is seeking a QAPI Specialist to advance clinical quality and patient safety through data-driven Performance Improvement Plans. The role compiles trends, audits, and reports on outcomes, infection control, patient satisfaction, and risk management, guiding staff with education and regulatory guidance to reduce variation in practice and improve scores.

Travel to branches within the US may be required; CT licensure is preferred; 3 years of certified home health

Qualifications

• 3 years of certified home health experience required.
• Knowledge of Value Based Purchasing, PDGM, CAHPS.
• CT licensure in field of specialty or eligible.
• Ability to design and develop performance measures and educate staff.

Responsibilities

• Develop and lead performance improvement plans using audit results.
• Compile trends and report quality data across patient care, safety, risk, and infection control.
• Support regulatory compliance and improve clinical documentation.
• Coordinate with PI teams and provide education to staff.
• Travel to branches as needed and communicate effectively across teams.
