Qualifications

  • 3 years of certified home health experience required.
  • Experience with Value Based Purchasing, PDGM, CAHPS-preferred.
  • CT Licensure in field of specialty
  • Knowledge of federal and state regulatory requirements
  • Ability to design and develop performance measures
  • Ability and willingness to travel to branches (regions) as needed
  • Excellent written and verbal communication skills

Responsibilities

  • Assist with improving clinical documentation and outcomes using a data-driven approach.
  • Compile trends and report quality data in patient care, safety, risk management, infection control, outcomes and customer satisfaction.
  • Lead development of Performance Improvement Plans.
  • Develop process improvement plans, record reviews, data tracking and analysis.
  • Ensure compliance with federal and state regulations and timely data submissions to payors and regulators.
