Job summary

Continuum Therapy Partners is seeking a part-time Physical Therapist for Meadow View Nursing Center in Berlin, PA. The role emphasizes collaborative care and patient-centered treatment planning in a nursing center setting.

The candidate must hold a Doctor of Physical Therapy (DPT), possess an active state license, and ideally have at least 1 year of PT experience. A $5k sign-on or a $10k retention bonus is offered.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state Physical Therapy License; must pass NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required

• 1 year of previous experience as a Physical Therapist preferred
