Job summary

Continuum Therapy Partners, LLC in Milwaukee, WI seeks a Physical Therapist to join a growing network of care facilities. You will evaluate and treat patients, develop individualized plans, and collaborate with multidisciplinary teams to ensure optimal outcomes.

The role requires a Doctor of Physical Therapy (DPT), active Wisconsin license, NPTE passing, and a preference for 1 year of PT experience. Full-time daytime schedule with comprehensive benefits and a $5k sign-on bonus is offered.

Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy License; NPTE required.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities
