Job summary

Continuum Therapy Partners, LLC in Berlin, PA, invites dedicated Physical Therapists to join a therapist-led growth team. The part-time daytime role focuses on delivering individualized treatment plans at Meadow View Nursing Center.

You will evaluate progress, document outcomes, educate patients and families on home programs, and collaborate with other disciplines. A DPT and current PA license are required; a 1-year experience is preferred, with sign-on or retention bonus options.

Qualifications

• DPT degree required.
• Current valid state PT license; NPTE must be passed.
• 1 year of PT experience preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
