Job summary

Continuum Therapy Partners, LLC is hiring a Physical Therapist for our Heritage Hall Big Stone Gap location in Big Stone Gap, VA. The role focuses on delivering patient-centered care and developing individualized treatment plans in collaboration with a multidisciplinary team.

You will evaluate patients, educate families on home exercise programs, and document progress while maintaining a high standard of clinical outcomes.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state PT license; NPTE required.
• 1 year of PT experience preferred.
• Evaluates, develops, and implements individualized treatment plans.
• Documents patient progress and adjusts care plans.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention
• Participates in ongoing patient assessment and modifies therapy accordingly.
• Collaborates with the team to optimize patient outcomes.

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required


• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)


• 1 year of previous experience as a Physical Therapist preferred


• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines


• Documents patient progress and adjusts plans of care based on clinical outcomes


• Educates patients and families on home exercise programs and injury prevention
