Continuum Therapy Partners, LLC is seeking a Physical Therapist to deliver patient-centered care at Heritage Hall Dillwyn, Dillwyn, VA. You will evaluate patients, develop and implement treatment plans, and collaborate with a multidisciplinary team to optimize outcomes.

Qualifications

• Doctor of Physical Therapy (DPT) degree is required.
• Current, unencumbered state PT license; NPTE must be passed.
• One year of PT experience is preferred.

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
