Job summary

Continuum Therapy Partners is seeking a Part-time Physical Therapist in Elroy, WI. A Doctor of Physical Therapy (DPT) degree is required, with an unencumbered Wisconsin PT license and NPTE, and about one year of PT experience preferred.

You will evaluate and develop individualized treatment plans in coordination with other disciplines and document patient progress to adjust care as needed. Educating patients and families on home exercise programs and injury prevention is also a key duty.

Qualifications

• DPT degree required.
• Current, unencumbered WI PT license; NPTE must be passed.
• 1 year of PT experience preferred.
• Evaluates, develops, and implements individualized treatment plans with other disciplines.
• Documents patient progress and adjusts plans of care based on outcomes.
• Educates patients and families on home exercise and injury prevention.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required

• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)

• 1 year of previous experience as a Physical Therapist preferred

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines

• Documents patient progress and adjusts plans of care based on clinical outcomes

• Educates patients and families on home exercise programs and injury prevention
