Job summary

Continuum Therapy Partners in Kewaunee, WI, is seeking a part-time Physical Therapist to join a company committed to patient-centered care and clinician-led collaboration.

Candidates must hold a Doctor of Physical Therapy (DPT), be license-eligible in Wisconsin and pass the NPTE, with at least one year of PT experience preferred; you will evaluate and implement individualized treatment plans and educate patients and families.

Qualifications

• DPT degree is required.
• Current, unencumbered Wisconsin PT license; NPTE must be passed.
• 1 year of PT experience preferred.
• Evaluates, develops, and implements plans of care with other disciplines.
• Documents patient progress and adjusts plans of care.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required

• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)

• 1 year of previous experience as a Physical Therapist preferred

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines

• Documents patient progress and adjusts plans of care based on clinical outcomes

• Educates patients and families on home exercise programs and injury prevention
