Job summary

Continuum Therapy Partners is seeking a Licensed Physical Therapist to join our team in Ohio. The role involves evaluating and treating patients, developing and adjusting plans of care, and educating families on home exercise programs and injury prevention.

Requirements include a Doctor of Physical Therapy degree, an unencumbered state license, and NPTE eligibility. A minimum of 1 year of PT experience is preferred. Full-time daytime schedule with multiple locations available in OH.

Qualifications

• Current, unencumbered state Physical Therapy License; NPTE required.
• DPT degree is mandatory.
• 1 year of PT experience preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans.
• Documents patient progress and adjusts plans of care based on outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required

• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)

• 1 year of previous experience as a Physical Therapist preferred

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines

• Documents patient progress and adjusts plans of care based on clinical outcomes

• Educates patients and families on home exercise programs and injury prevention
