Job summary

Strive Health seeks a Network Engagement Partner to maintain and grow provider and network relationships across contracted markets. You will collaborate with nephrologists, hospitals, and PCPs to ensure CKD/ESRD patients receive high-quality, continuous care, while supporting new market launches and provider incentive programs.

You will travel as needed to meet stakeholders, educate staff, and drive engagement with performance targets. The Sr.

Qualifications

• High school diploma or GED equivalent.
• 6+ years combined of related education, experience, or certification.
• Must have reliable transportation and active driver's license for travel within assigned region.
• Ability to travel and be on-site as needed.
• Internet connectivity with min speeds 3.8Mbps/3.0Mbps (latency < 60 ms).

Responsibilities

• Maintain external provider and network relationships in contracted markets.
• Grow provider network as Strive launches new markets.
• Use data-driven approach to outline network outreach strategy with market directors.
• Review data and reporting with providers to improve outcomes.
• Collaborate with business development to identify new partnership opportunities.
• Drive engagement, quality of care, and utilization targets across practices.
• Engage, contract, and manage practices in provider incentive programs.
• Meet with stakeholders; travel may be required for in-person connections.
