Job summary

Strive Health is seeking a Network Engagement Partner to manage external provider relationships in markets where we deliver services, focusing on CKD/ESRD patient care and continuity of care. You will establish and grow relationships with provider groups as Strive launches new markets.

The role requires strong collaboration with leaders to drive engagement, quality of care, and utilization. Expect travel to meet stakeholders and deliver on program objectives.

Qualifications

• High school diploma or GED equivalent.
• 6+ years combined of related education, experience, or certification.
• Active driver\'s license and ability to travel across an assigned region.
• Willingness to travel and be on-site to meet business needs.
• Internet Connectivity – Min Speeds: 3.8Mbps/3.0Mbps (up/down).

Responsibilities

• Maintains relationships with provider groups using an account management approach; completes provider orientation for applicable product lines.
• Analyzes utilization, claims, demographics to outline network and outreach strategy.
• Uses data-driven approach to support initiatives reducing total cost of care.
• Reviews data with provider groups to improve operational and clinical outcomes.
• Collaborates with business development to identify partnership opportunities in markets.
• Drives performance of portfolios, meeting engagement and quality targets; supports new workflows.
• Engages, contracts, and manages practices in provider incentive programs where applicable.
• Sits in-person with stakeholders to advance priorities; travel may be required.
