Optimal Care is seeking a Physician to deliver advanced primary and specialty care to geriatric patients, often in homes or care settings, with the autonomy to focus on thoughtful, patient-centered care that improves comfort and quality of life. You will conduct comprehensive medical evaluations, diagnose and manage conditions, and coordinate care with patients, families, and interdisciplinary teams.

You’ll provide leadership for Nurse Practitioners and Physician Assistants and mentor fellow

Qualifikationen

• MD or DO degree (Physician) required.
• Current unrestricted State medical license.
• Board certification in family practice, internal medicine, geriatric or palliative care.
• Reliable transportation and valid driver’s license.

• Perform comprehensive medical evaluations and manage acute and chronic conditions.
• Develop individualized treatment plans and coordinate care with patients, families and interdisciplinary teams.
• Provide medical leadership and oversight for NP/PA clinicians and mentor colleagues.
• Improve patient outcomes and reduce unnecessary hospitalizations.
