Job summary

Optimal Care is seeking a Physician to provide advanced primary and specialty care to geriatric and medically complex patients, often in homes or care settings, with autonomy to deliver thoughtful, patient-centered care that improves comfort and quality of life.

You will conduct comprehensive evaluations, diagnose and manage conditions, develop treatment plans, and coordinate care with families and healthcare teams, while mentoring physicians, NPs, and PAs for high-quality, coordinated care.

Qualifications

• MD or DO required.
• Current unrestricted state license to practice medicine.
• Board certification in family practice, internal medicine, geriatric or palliative care.

Responsibilities

• Provide advanced primary and specialty care to geriatric and medically complex patients, often in home or care settings.
• Lead medical oversight for Nurse Practitioners and Physician Assistants and mentor clinicians.
• Coordinate care with patients, families, and interdisciplinary teams to improve outcomes.
