Job summary

Health Wealth Safe in Louisiana seeks a licensed Physical Therapist to provide personalized, one-on-one care to older adults in private residences or senior living communities. You will assess mobility, develop care plans, deliver evidence-based treatments, and document in EMR systems while collaborating with an interdisciplinary team.

Qualifications

• valid PT license or eligibility in the practice state
• degree from an accredited PT program
• proficiency in basic computer use and EMR systems
• current CPR certification

Responsibilities

• Conduct assessments and develop care plans for mobility and independence
• Deliver evidence-based physical therapy treatments
• Document care in EMR systems and collaborate with interdisciplinary teams
• Educate patients, caregivers and staff to promote safety and aging in place
• Support telehealth or virtual care to enhance access

Job description

• Valid Physical Therapy license or eligibility in practice state

Physical Requirements

• Medium physical effort, including lifting up to 50 lbs occasionally
• Assist with patient transfers, repositioning, and therapeutic activities
• Extended standing, walking, climbing stairs, and driving to patient locations
• Ability to wear PPE when necessary
