Responsibilities

Key Responsibilities Include

• Conducting evaluations and developing individualized treatment plans
• Delivering evidence-based therapy to enhance mobility, independence, and wellness
• Documenting patient progress accurately within our EMR system
• Collaborating with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educating patients, caregivers, and staff on safety and functional aging
• Providing telehealth and virtual care to extend access and flexibility

Qualifications And Skills

• Valid Physical Therapy license or eligibility to obtain one
• Degree from an accredited Physical Therapy program
• Proficiency in computer and EMR systems
• Current CPR certification

Physical demands

Ability to perform moderate physical effort, assist with patient transfers, reposition patients, and drive to various locations, while wearing PPE when necessary.
