Job summary

Alpha Home Health and Hospice is seeking a dedicated Physical Therapist to provide one-on-one in-home care to older adults across Seattle. You will assess mobility, develop personalized plans, and deliver evidence-based therapies aimed at improving independence and safety at home.

Collaborate with PTs, OTs, SLPs, and fitness staff, educate patients and caregivers, and document progress in EMR. Telehealth options and flexible scheduling support optimal patient outcomes.

Qualifications

  • Valid Physical Therapy license or state eligibility.
  • Degree from an accredited Physical Therapy program.
  • Proficiency with EMR systems and computers.
  • Current CPR certification.

Responsibilities

  • Conduct patient evaluations and develop tailored care plans.
  • Deliver evidence-based therapies to improve mobility and independence.
  • Document progress in electronic medical records.
  • Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
  • Educate patients, caregivers, and staff to promote safety at home.

Responsibilities include conducting evaluations, developing tailored care plans, delivering evidence-based treatments, and documenting progress within electronic medical records. Physical & Practical Requirements
  • Ability to exert medium physical effort, including lifting up to 50 lbs occasionally
  • Capability to assist with patient transfers, repositioning, and therapeutic activities
  • Willingness to drive to patient locations and wear PPE as needed
