Job summary

MEP Health is seeking a licensed Physical Therapist to deliver in-home and senior living care, improving mobility and independence for older adults in New Jersey. You’ll conduct evaluations, create individualized plans, and provide evidence-based treatments in clients’ homes and community settings.

Qualifications

• PT license or eligibility in practice state.
• Degree from an accredited Physical Therapy program.
• Proficiency with computers and EMR systems.
• Current CPR certification.

Responsibilities

• Provide one-on-one therapy sessions in private residences and senior communities.
• Conduct evaluations, develop individualized care plans, and deliver evidence-based treatments.
• Support patient wellness, functional independence, and longevity through therapy.
• Document care accurately in electronic medical records.
• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Utilize telehealth and virtual care to enhance access and flexibility.
