Job summary

Preferred Home Health Care & Nursing Services is seeking a licensed Physical Therapist to provide in-home, senior living, and virtual care to older adults in New Jersey. You will focus on improving mobility, independence, and quality of life while coordinating care with an interdisciplinary team.

The role involves conducting evaluations, developing individualized plans, documenting progress in EMR, and educating patients and caregivers to promote aging in place.

Qualifications

• Valid Physical Therapy license or eligibility in practice state(s).
• Degree from an accredited Physical Therapy program.
• Proficiency in basic computer and EMR systems.
• Current CPR certification.

Responsibilities

• Conduct one-on-one evaluations and develop individualized treatment plans based on evidence-based practices.
• Deliver therapy sessions to support patient wellness, mobility, and functional independence in various settings.
• Document patient progress accurately within electronic medical records, adhering to regulatory standards.

• Utilize telehealth and virtual care to enhance access and continuity of care.

Physical Requirements
