Job summary

Proforma Health is seeking a Licensed Physical Therapist to provide personalized in-home, senior living, and virtual care to older adults. You will evaluate patients, craft treatment plans, and deliver therapy to enhance mobility and overall wellness.

Responsibilities include EMR documentation, collaboration with interdisciplinary teams, caregiver education, and utilizing telehealth as appropriate. Flexible scheduling with opportunities for full-time, part-time, or PRN roles.

Qualifications

• Valid PT license in good standing.
• DPT/degree from an accredited PT program.
• Proficiency with EMR systems and basic computer skills.
• Current CPR certification.

Responsibilities

• Conduct evaluations and develop individualized treatment plans.
• Deliver evidence-based therapy to improve mobility and independence.
• Document care in the EMR and collaborate with interdisciplinary teams.
• Educate patients and caregivers; incorporate telehealth when appropriate.
• Travel to patient locations for in-home and facility visits.

Role:
