Job summary

Northshore Home Health Care is seeking a licensed physical therapist to provide in-home and community-based physical therapy services. The role emphasizes mobility, aging-in-place outcomes, and evidence-based treatments.

You will work with diverse patients in their homes and senior living settings. You will create individualized treatment plans, document progress in EMR, and collaborate with an interdisciplinary team while educating patients and caregivers on safe, independent living.

Qualifications

• Valid Physical Therapy license or eligibility in applicable state(s).
• Degree from an accredited program (DPT preferred).
• Proficiency with electronic medical records (EMR) and computer use.
• Current CPR certification.

Responsibilities

• Deliver one-on-one therapy sessions in patients' homes or senior facilities.
• Develop and document individualized care plans in EMR.
• Collaborate with an interdisciplinary care team.
• Educate patients, caregivers, and staff on safety and aging in place; use telehealth when appropriate.

Role Overview:

This position involves providing personalized, one-on-one physical therapy to older adults in their homes or senior living communities, supporting mobility, independence, and quality of life through evidence-based treatments and evaluations.

Responsibilities Include

• Delivering therapy sessions in various settings, including private residences and senior facilities

• Utilizing telehealth and virtual care to extend access and flexibility

Qualifications And Skills

Physical Requirements:

Ability to perform moderate physical effort, assist with patient transfers, stand and walk for extended periods, climb stairs, and drive to patient locations. PPE use is required when necessary.
