Job summary

Rock Health is seeking a Licensed Physical Therapist to provide personalized, in-home and senior living care to older adults, with opportunities for virtual care support. Deliver one-on-one PT in homes and communities, conduct evaluations, and create individualized treatment plans to enhance mobility and independence.

You will document care in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and may work full-time, part-time, or PRN with competitive compensation

Qualifications

• Valid Physical Therapy license or eligibility in the practice state.
• Degree from an accredited PT program.
• Proficiency with computers and EMR systems.
• Current CPR certification.

Responsibilities

• Deliver one-on-one PT in homes and senior communities.
• Evaluate patients, develop treatment plans, and provide therapies.
• Improve mobility, independence, and wellness.
• Document care in EMR per regulatory standards; collaborate with PTs, OTs, SLPs.

Role:

Responsibilities

• Deliver one-on-one physical therapy in various settings including private residences and senior communities
• Perform evaluations, develop individualized treatment plans, and provide evidence-based therapies
• Enhance patient mobility, independence, and overall wellness
• Document care accurately within electronic medical records following regulatory standardsCollaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Utilize telehealth/virtual care to extend access and flexibility when appropriate

Qualifications & Skills

• Valid Physical Therapy license or eligibility in the practice state
• Degree from an accredited PT program
• Proficiency with computers and EMR systems
• Current CPR certification

Physical Requirements:

Ability to perform moderate physical activities including patient transfers, mobility assistance, and driving to patient locations. Must meet physical demands of community-based care and wear PPE when necessary.
