Job summary

LiveWell Homecare Agency, LLC is seeking a Licensed Physical Therapist to provide in-home, senior living, and virtual care to older adults.

You will conduct evaluations, develop individualized treatment plans, and deliver evidence-based therapy to improve mobility and independence. Documentation in EMR and collaboration with interdisciplinary teams are essential.

Qualifications

• Valid PT license in the United States.
• Degree from an accredited Physical Therapy program.
• Computer proficiency.
• CPR certification.
• Ability to meet physical demands such as lifting, repositioning, and driving.

Responsibilities
