Job summary

Cherry Hill Free Clinic is seeking a Licensed Physical Therapist to provide in-home and community-based care to older adults in Myrtle Beach, with optional telehealth services. You will perform evaluations, create individualized treatment plans, and deliver evidence-based therapy to improve mobility and independence.

Responsibilities include documenting sessions in EMR, collaborating with interdisciplinary teams, educating patients and caregivers, and driving to patient locations when needed.

Qualifications

• Valid state license in good standing.
• DPT or equivalent from an accredited program.
• Current CPR certification.
• Ability to lift up to 50 lbs and perform transfers.
• Willingness to drive to patient locations.
