Job summary

Seacrest Behavioral Health seeks a Licensed Physical Therapist to deliver in-home and senior living care, with options for telehealth. You will evaluate patients, design individualized plans, and provide evidence-based therapy to maintain mobility and independence for aging adults.

You’ll document progress in EMR, educate families, and coordinate with an interdisciplinary team to enhance safety and aging-in-place strategies. Travel to patient locations is required as needed.

Qualifications

• Valid PT license or eligibility.
• Degree from an accredited PT program.
• Proficiency with EMR and basic computer skills; CPR certification.
• Must meet physical demands of community-based care (lifting, repositioning, driving).
