Job summary

DayToDay Health is seeking a Licensed Physical Therapist to provide personalized, one-on-one care to older adults in private homes and senior living communities.

You will conduct evaluations, create individualized plans, and deliver evidence-based therapies to improve mobility and independence, with EMR documentation and telehealth as appropriate.

Qualifications

• Valid PT license
• Degree from an accredited program
• Proficiency in EMR and computers
• Current CPR certification

Qualifications & Skills

Ability to perform physical tasks including lifting, patient transfers, repositioning, and driving to patient locations. Physical stamina for extended periods of mobility and assisting patients.
