Ascension Health is seeking a Rehabilitation Physical Therapist at Franklin Hospital in Franklin, WI. The role focuses on patient assessments, developing and revising care plans, and delivering PT interventions to improve patient outcomes.

You will also supervise PT assistants and students within the Rehabilitation department. The position requires a Bachelor's degree (Master's/Doctorate preferred) and WI-credentialed PT/RPT with BLS within 30 days of hire.

Formación

• BLS Provider required within 1 month of hire date or transfer date.
• Licensed Rehab Therapist (RPT) credentialed from the Wisconsin Physical Therapy Examining Board prior to hire date or transfer date.
• Physical Therapy (PT) credentialed from the Wisconsin Physical Therapy Examining Board prior to hire date or transfer date.
• Bachelor's degree required; Master's or Doctorate preferred.

Responsabilidades

• Performs initial and ongoing assessments of patient's condition.
• Establishes and evaluates a plan of care involving patient and family.
• Performs therapy interventions using standard physical therapy techniques.
• Oversees activities of physical therapy assistants, students and other support personnel.

Your future role at a glance

• Location: Franklin, WI
• Facility: Franklin Hospital
• Department: Rehabilitation
• Schedule: PRN | weekend shifts 2 per month, and 1 holiday per year

How you’ll make an impact in this role

• Performs initial and on-going assessments of patient's condition.
• Establishes, revises and evaluates a plan of care which is appropriate to problems identified and involves the patient/family.
• Performs therapy interventions utilizing standard physical therapy techniques and skills as appropriate for the condition of the patient.
• Oversees activities of physical therapy assistants, students and other support personnel.

What minimum requirements you’ll need

Licensure / Certification / Registration

• Required Credential(s):
• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
• Licensed Rehab Therapist specializing in Physical Therapy (RPT) credentialed from the Wisconsin Physical Therapy Examining Board obtained prior to hire date or job transfer date. Physical Therapy (PT) credentialed from the Wisconsin Physical Therapy Examining Board prior to hire date or job transfer date.

Education

• Bachelor's degree required. Master's OR Doctorate degree preferred.
